Job Description

We are seeking a Clinical Nurse who will support the General Internal Medicine Resident practice at the Johns Hopkins Outpatient Center (JHOC).


Specific Duties & Responsibilities

  • Coordinates care for a group of complex patients achieve clinical and organizational outcomes with an emphasis on population health management.
  • Provides evidence-based care based on nursing process, policies, procedures, and protocols of the Johns Hopkins Hospital.
  • Provides guidance to other team members to promote quality, safety, and excellence in care.
  • Performs transitional care management calls for practice patients discharged from hospitals and emergency departments in the area and determines the need for short-term outpatient follow-up. Assesses patient needs during those calls and connects patients with appropriate resources and referrals.
  • Provides direct patient care when needed for point of care testing, medication/vaccine administration, and other duties as assigned.
  • Provides patient education for self-care chronic disease management, such as, diabetes education (in person, by phone, or via MyChart).
  • Interfaces with payors for prior-authorizations of vital patient medications and equipment.
  • Triages phone calls and electronic advice requests from patients and determines need for medical assessment.
  • Helps complete forms (e.g., disability, FMLA, mobility) requiring basic health history.
  • Identifies care continuity needs of patients; generates and pends orders to primary providers for signature.
  • Actively participates in Comprehensive Unit Based Safety Program (CUSP) initiatives.
  • Helps lead quality improvement initiatives that are focused around outcomes measures that are relevant to primary care.
  • Works collaboratively with a multidisciplinary health care team to optimize clinic performance and workflows.
  • Works with healthcare team to develop and maintain concise care plans for chronic disease management. Tracks patient progress and care received to ensure standards of care, particularly for identified chronic conditions.
  • Utilizes aggregated dashboard data to manage a population of patients with respect to clinical outcomes and goals of care.
  • Identifies care gaps for patients, develops procedures to address them systematically, and works collaboratively with healthcare team to close gaps for individual patients. Responsible for other clinical duties as assigned.

Special Knowledge, Skills & Abilities

  • Expected to learn the formal and informal departmental goals, standards, policies and procedures which may include some familiarity of other departments within the school/division. Is sensitive to the interrelationship of both people and functions within the department.
  • On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of service within the functional area.
  • Exchanges non-routine information using tact and persuasion as appropriate requiring good oral and written communication skills.
  • Knowledge of nursing process; social, behavioral, biological, and physical sciences; safety and scientific inquiry process and application to the skill performance; leadership and management as normally acquired through completion of a bachelors program in nursing. Knowledge of current health care environment.
  • Knowledge of general physiologic monitoring and patient care equipment such as defibrillator and glucometer monitor.
  • Successfully completes all training courses required for access to Epic and other clinical software systems.
  • Requires effective use of resources and cost awareness.
  • Ability to analyze data to synthesize and summarize information and to make judgments regarding patient care. Highly effective verbal communication and interpersonal skills to establish working relationships that foster optimal quality patient care.
  • Ability to use a computer keyboard to input and obtain data.
  • Ability to operate personal computer to access email, computerized scheduling and documentation systems, and other basic office support software including Microsoft Office applications.
  • Ability to apply principles of evidence-based practice, scientific process, and service excellence in practice. Ability to interact successfully with the public. Ability to perform effectively despite sudden deadlines and changing priorities. Ability to maintain personal composure in high stress situations.
  • Demonstrated interpersonal, written and verbal communication skills are required to service a diverse customer base.
  • Ability to demonstrate and convey a favorable image of the organization and to conform to proper standards of professional dress, attitude and demeanor. Ability to demonstrate a high level of interpersonal skills required to interact with patients, patients? families/visitors and clinical staff.
  • Ability to perform with a high degree of accuracy and with meticulous attention to detail. Demonstrate a strong ability to use initiative and judgment and to identify, analyze and solve problems.
  • Ability to demonstrate self-direction and initiation. Ability to work independently and as a part of a team.

Physical Requirements

  • Possible exposure to communicable diseases, hazardous materials, radiation, and pharmacological agents.
  • Ability to move about or sit for an extended period of time.
  • Work requires lifting of 35lbs without assistance, unless special accommodations are in place.
  • When lifting weight above 35lbs assistance or assistive lifting devices should be used.
  • Reaching by extending hand(s) or arm(s) in any direction.
  • Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard.
  • Communication skills using the spoken word.
  • Bilingual Spanish speaker preferred.
  • Ability to see within normal parameters. Ability to hear within normal range.




Minimum Qualifications

  • Individual must be a registered nurse, licensed in the State of Maryland or state where practicing. Bachelor's degree in nursing or related discipline preferred.
  • Must maintain active licensure throughout employment.
  • Experience with Epic preferred.
  • Bilingual Spanish speaker preferred.
