About the Role
We are seeking a dedicated Licensed Mental Health Counselor to join our collaborative care team. In this role, you'll work directly with patients as well as alongside primary care providers and health advocates to create, implement, and manage personalized care plans. Your expertise in behavioral health will be essential as you conduct biopsychosocial assessments, identify barriers to care, and assist patients in navigating the healthcare system. As an integral part of the team, you will also serve as the primary point of contact with external community organizations to ensure patients adhere to their care plans.
Key Responsibilities:
Consult directly with patients—both virtually and in-person (including in the community and patients' homes)—to understand their social needs and connect them with appropriate resources.
Collaborate with the care team to develop individualized care plans based on each patient's needs and goals.
Provide guidance and support to the interdisciplinary team to ensure comprehensive patient care.
Conduct psychosocial evaluations, including mental status exams, and translate clinical impressions into actionable care plan goals.
Offer crisis intervention and provide short-term, evidence-based therapy as needed.
Build strong relationships with community organizations to support patient care and ensure ongoing adherence to treatment plans.
Field & Remote Work Requirements:
This position involves field-based patient care, with approximately 60% of your time spent traveling to patient visits in Syracuse, NY, and surrounding areas. The remaining time will be remote work. You will have the flexibility to travel via personal transportation or utilize our mobile unit for patient visits.
Compensation:
$60,000 - $70,000

Qualifications:
Active license to practice as a Licensed Mental Health Counselor
1 to 3 years of post-master's experience working with the behavioral health population, including field-based or community-based care experience
Knowledge and experience in assessing and intervening with individuals with Serious Mental Illness (SMI) and Substance Use Disorders (SUDs)
Experience conducting psychosocial assessments and creating care plans
